The module displays a form that has been created by eXtroForms commercial. Due to the limited space in a module position, the 5 different eXtroForms positions have been removed and all fields will be displayed as a list.

Please note: This module is not compatible with the free version of eXtroForms, it can only be downloaded and used if you have purchased the commercial version of eXtroForms.
